This is a request for readmission to the program/major that is currently on your record.
This form is only for students who left their program in good standing. If you were dismissed from your program, the Petition for Reinstatement must be submitted to BUGrad@bradley.edu.
If you want to change your current program/major, you must submit a new application for graduate admission using the Graduate Application link.
Your readmission does not guarantee that you will be eligible for financial assistance. You must meet federal, state and Bradley University polices of satisfactory academic progress.

All financial and academic holds must be satisfied before your readmission will be finalized. We cannot process your application without your signature.
